Patient Compliance Checklist: Staying on Track with Care, Without Feeling Overwhelmed

Use this guide when talking with your care team, or supporting someone else through illness.

Understand Your Plan
What is my diagnosis, and how does this treatment help?
What are the side effects of my medications?
Can we simplify or adjust this plan if needed?

Address Barriers Early
What if I can’t afford this medication?
What if I forget or struggle to take it correctly?
Can you help me with transportation or home support?

Use the Right Tools
What apps or systems can help me stay on track?
Is there a caregiver or case manager who can check in?
How can I follow up if something changes?

Keep It Human
What’s most important to me in my daily life and health?
How can we make this plan work for me?
Can I bring someone with me to help me remember and ask questions?

Final Reminder: Compliance = Collaboration.
It’s not about “doing it perfectly”—it’s about working together for a plan that fits.
