A simple guide to living safely, confidently, and on your own terms

Daily Living & Movement

  • I moved my body today in a simple, safe way (walking, stretching, light activity)
  • I practiced balance or strength, even briefly
  • I listened to my body and rested before becoming overtired

 

Home Safety

  • My walking paths are clear and free of clutter or loose rugs
  • I have adequate lighting, especially at night
  • Frequently used items are within easy reach (no overreaching or climbing)

 

Connection & Support

  • I connected with someone today (call, text, visit)
  • I have at least 2–3 people I can reach out to if needed
  • I allowed someone to help me in a small way if needed

 

Routine & Organization

  • I followed a simple daily routine (meals, medications, activity)
  • I took my medications as scheduled
  • I planned at least one small task for the day (laundry, meal prep, errands)

 

Health & Prevention

  • I paid attention to any new symptoms or changes
  • I stayed hydrated and ate regular meals
  • I kept up with appointments or made note of anything I need to schedule

 

Using Support Tools

  • I used tools that make life easier (pill organizer, reminders, assistive devices)
  • I am open to trying new tools or small changes that improve safety
  • I asked for help or information when something felt difficult

 

Weekly Check-In

  • Did I stay active most days this week?
  • Did I stay connected with others?
  • Is there anything in my home that feels unsafe or difficult?
  • What is one small change I can make this week to stay independent?
