Unsafe Hospital Discharge Checklist: Are You Ready to Go Home?

Use this checklist before leaving the hospital to make sure your discharge is safe, clear, and supports your recovery—

not risks it.


1. Do I Understand My Condition and Care Plan?

  • I understand my diagnosis and what was treated during this stay
  • I know what symptoms should be improving—and what should not
  • I’ve had a chance to ask questions and get clear answers
  • I do not feel rushed or confused about leaving

 

2. Do I Understand My Medications?

  • I have a complete list of all medications (new, changed, and stopped)
  • I know what each medication is for
  • I understand when and how to take them
  • I know possible side effects and what to watch for
  • I know where and how to get my prescriptions filled

 

3. Do I Have Clear Written Instructions?

  • I received written discharge instructions
  • I understand wound care, diet, and activity restrictions
  • I know what symptoms mean I should call for help or return to the hospital
  • I know who to contact if I have questions

 

4. Is My Follow-Up Care Arranged?

  • Follow-up appointments are scheduled (or I know exactly how to schedule them)
  • I know which doctor I need to see and when
  • I understand any needed lab work, therapy, or additional testing
  • I have transportation arranged if needed

 

5. Is My Home Situation Safe?

  • I have the help I need at home (family, caregiver, or support)
  • I feel physically able to manage daily activities (walking, bathroom, meals)
  • I have necessary equipment (walker, oxygen, wound supplies, etc.)
  • My home environment feels safe for recovery

 

6. Do I Feel Ready to Leave?

  • I feel stable enough to go home
  • My pain or symptoms are controlled
  • I feel confident managing my care
  • I do not feel pressured to leave before I’m ready

 

If Something Feels Off

  • I have spoken up about concerns before discharge
  • I know I can ask to speak with a nurse, case manager, or patient advocate
  • I understand that it is okay to ask for more time if I feel unsafe

 

Final Reflection

  • Do I feel prepared—or just ready to leave?
  • Do I feel supported—or alone in this next step?
  • Do I understand what comes next—or am I guessing?

 

A safe discharge is not about speed—it’s about stability, clarity, and support.
You deserve to leave the hospital feeling prepared, not uncertain.
